Our Home Care Aides are an integral part of our team, assisting participants at home and participating in the care planning process as an equal part of our Interdisciplinary Team (IDT). Reporting to the Home Care Manager, the Home Care Aide provides safe personal care services to WelbeHealth participants in their home based on a plan and instructions.

Nursing staff provide personal care and activities of daily living (ADL) to WelbeHealth participants at home, including bathing, dressing, grooming, dressing, toileting, and transferring participants when participants exchange.
Assist participants with activities of daily living (IADL), including but not limited to laundry, light work, shopping, etc. Measure and record participants’ vital signs as directed
Assist in preparing and delivering snacks to participants’ homes to ensure participants are adequately fed and hydrated.
Assist participants in getting in and out of the home and staying comfortable and acting as a medical companion at assigned transportation and medical appointments
As a caregiver, you will be fully trained to assist the Rehabilitation Department in implementing a home-based rehabilitation program as outlined in the care plan developed by the IDT.
Provide translation services as needed if you meet the requirements
Job Requirements:
High school diploma or equivalent
Basic Life Support (BLS)/First Aid certification currently required
Candidates with a Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
Bilingualism preferred
Must be registered with the Department of Health Home Care Registry on start date
